Insurance & Benefit Enrollment Assistance

Citrus Health Network has Certified Application Counselors to assist individuals and families in applying for Medicaid and KidCare, as well as other benefits such as food stamps and temporary cash assistance.  Citrus staff can also help individuals and families navigate the process of insurance enrollment through the Affordable Care Act Marketplace. 

What to Bring to Your Appointment

Information needed for every applicant and household member:

  1. Full Name
  2. Date of Birth
  3. Social Security Card
  4. Employer and income information for all family members
  5. Policy name and numbers for any current health insurance
  6. Information regarding health insurance offered by an employer
  7. Personal e-mail and corresponding password
  8. Certificate of Citizenship for naturalized citizens, or Permanent Resident Card, Employment Authoirzation Card, or I-94 for non-citizens.
